Microwave ablation of adenomyosis – a conservative uterus-preserving procedure
What is a microwave ablation of the uterus?
Microwave ablation (MWA) is a specialized technique, which is only recommended in women who desire to preserve their uterus. This is a technique that benefits young women the most.
Dr Jay Mehta and team perform this technique with a great amount of expertise because this is one of those surgical skills that requires extremely good laparoscopic surgery to be done simultaneously with the procedure.
The Microwave Ablation Procedure
The ablation device is a simple device which runs on the principle of microwave ablation where a specific heat signal is generated through a dedicated probe, which goes through the adenomyotic tissue under ultrasound as well as laparoscopic guidance.
In order to perform this technique correctly, one has to clearly remember that all the adhesions which are present in the pelvis have to be cleared meticulously. This predominantly involves clearance of all the bowel as well as the bladder adhesions.
The microwave ablation (MWA) would work best in cases where there is presence of diffuse Adenomyosis. A calculated heat signal through the microwave probe is then delivered under the guidance of the ultrasound and ablation is carried out till a circumference of approximately 2.5 cm is ablated.
This procedure is not going to be effective in women who are having focal adenoma of the outer myometrium (FAOM). In cases where there is presence of a nodule of deep endometriosis, one has to clear off the entire nodule, prior to starting the micro ablation procedure.
It usually takes approximately three months for the patient to experience volume reduction with the help of microwave ablation, and for these three months, a specific medical management is initiated for the patient which is decided on a case to case basis by Dr Jay Mehta. Usually after the first hundred days of this procedure, repeat ultrasound is done for the patient in order to assess the significant volume reduction after this ablation.
In carefully selected cases, patients who are desirous of having fertility will also greatly benefit. However, one of the most important criteria for fertility benefit is the preservation of the endometrial junction, which can be assessed easily prior to doing the surgery. As a result of this, patients who desire a fertility outcome should have an expert consultation with Dr Jay Mehta and team across all locations in India to actually seek guidance if they will genuinely benefit from the procedure or not.
One must also remember that the microwave ablation probe is a single use device and it is an expensive procedure to do as far as the Indian economic settings are concerned .
Currently, the microwave ablation is being practiced by Dr Jay Mehta and team in Mumbai and patients from across India, who are suffering from Adenomyosis and endometriosis, are called to Mumbai for doing this procedure.
