What is endometriosis?
Achieving pregnancy may be more difficult if you have endometriosis. The endometrium is the innermost layer of the uterus, and it is found in its usual location. Endometriosis occurs when the endometrial tissues overgrow within or outside the uterus.
It is termed “deep endometriosis” when it grows in other areas, over the ovaries, fallopian tubes, or behind the uterus. Hence, infertility affects 30% of women with endometriosis. This article aims to teach you how you would confirm if you or anyone else has endometriosis.
Where does this affect?
The ovaries are the site of endometriosis enlargement, referred to as a chocolate cyst. A chocolate cyst is a condition that prevents menstrual blood from passing through the uterus. As a result, the blood present in the uterus turns brown instead of red. This mainly happens because there is a prolonged standing of blood in that region and as a result of this, there is altered chemicals of the blood product which typically changes the colour of blood from red to brown.
Always remember, pain and infertility are the two major symptoms of endometriosis. Secondly, in the case of deep endometriosis, the endometrial tissue can also grow behind the uterus (rectum). The other commonly affected organs include the vaginal canal or cervix and the fallopian tubes.
See major majority of the women are commonly going to get worried because when they read their MRI report or an ultrasound report, they end up reading stuff like Adherent to the bowel, bowel nodule, and tethered bowel. All of this is practically fancy vocabulary, but it basically indicates an underlying disease process where there is Deep endometriosis.
Has a woman, you should also understand that endometriosis can also spread to extra pelvic locations and as a referral unit for endometriosis in India, we end up treating a lot of endometriosis across all the extra pelvic locations which is predominantly going to be in the scar site, umbilicus, perineum, diaphragm and even lungs.
Symptoms of Endometriosis
- Painful period
- Dysmenorrhea is a pain in the lower abdomen that occurs soon before or during menstruation. It could be minor discomfort for some, or it could also be serious for others. (25-35 age women)
- Consult your doctor immediately as they may recommend sonography. Ultrasound sonography gives a detailed picture of the uterus’ condition. It also helps to identify the form of endometriosis and the extent of its spread. It can help confirm the diagnosis of the following conditions:
- Chocolate cyst
- Deep endometriosis nodule
- Abnormalities in the ovaries
Hence, one of the most common diagnostic modalities is ultrasound sonography. It is the most effective test for determining whether you have endometriosis.
Though many tend to believe that ultrasound may not be as good as doing a detailed MRI for the identification of endometriosis. One has to crystal clearly understand that MRI is a static test where is ultrasound is a dynamic test doing ultrasound, especially even done by somebody who is an absolute expert in mapping of the disease is in fact, better in order to map the deep nodules for endometriosis along with the pain score at that particular location.

What is laparoscopic, and how does it work?
When a couple cannot conceive for a long time, doctors frequently recommend this to rule out underlying medical issues. Now, this is predominantly labeled as diagnostic Hystero laparoscopic procedure.
Once upon a Time, this was considered to be the gold standard for diagnosis of endometriosis, especially Deep endometriosis and perennial endometriosis. However, in modern times, a good ultrasound has replaced the value of a diagnostic laparoscopy.
Because endometriosis is a disease which predominantly affects the deep tissue. One has to note that it is also commonly associated with a condition called as Adenomyosis. As a result of this, it can cause extremely painful periods which is called as dysmenorrhea.
Endometriosis, especially in the deep tissues can also associated with symptoms like dyspareunia, which is having painful intercourse and dyschezia which means having pain while defecation.
It is also provided to understand that approximately 7 to 10% of women who are experiencing extremely severe endometriosis may or may not have any associated pain of endometriosis.
Now I would like to explain why does endometriosis cause so much pain in some women and why it is absolutely asymptomatic in women who are also having extensive disease…? This is predominantly because the endometriotic tissue secret enzyme called as PGE2 and this is predominantly responsible for the heavy amount of pain in the pelvis. However, PGE2 is an extremely morphological and polymorphic substance which is secreted and as a result of which it is impossible to predict, which woman would experience what amount of pain.
Many women who tend to be asymptomatic, often ignore the disease, thinking that they have no pain. However, these are the women who required to be getting operated the first.
As we speak to each other, as of 2025 radical excision in the hands of an expert endometriosis, surgeon remains the treatment of choice.
Conclusion
If you experience any of the symptoms listed above, you may have endometriosis. To identify this disease, you’ll need to undertake clinical tests and sonography. You should be informed of the facts and not dismiss the issue.
You should go to the doctor and get the proper treatment for your endometriosis. Many patients rely solely on pain relievers for such symptoms, which is entirely incorrect. Correct diagnosis can help you avoid further complications, and you may soon get a piece of good news!
