Bladder Endometriosis

Deep Endometriosis of the Bladder is a relatively uncommon disease that affects the bladder, particularly the Trigone region. It may coexist with other nodular endometriosis growths in areas such as the Pouch of Douglas, bowel (intestines), ureter, and parametrium.
Although rare, abnormal locations outside the pelvis like scar tissue, the umbilicus, and the diaphragm can also be affected.
Some rare instances have been seen of isolated Endometriosis affecting the bladder.
It is important to differentiate Deep Endometriosis of the Bladder from typical endometriosis, as it is characterized by a higher presence of fibrosis and minimal glands and stroma.
Deep Endometriosis of the Bladder can commonly coexist with Ovarian Endometriosis, also known as a chocolate cyst.
Symptoms & Diagnosis of Bladder Endometriosis
Patients with Deep Endometriosis of the bladder may experience unusual symptoms such as pain while urinating or even bleeding during urination. Over time, the pain may persist despite medication.
In cases where the deep Endometriosis is near the ureter, patients may experience hydronephrosis, leading to loin pain due to kidney pressure changes.
The incidence of Deep Endometriosis of the bladder at a young age is gradually increasing, thanks to advancements in diagnostic methods.
High-quality ultrasound technology is especially beneficial in this regard. The examination typically involves a partially filled bladder for optimal results.
Ultrasound remains the best diagnostic method for Deep Endometriosis of the Bladder. However, this process needs to be conducted by a specialized unit proficient in this technique and correct disease mapping.
In our clinic, Dr. Jay Mehta & his colleagues follow a stepwise protocol resulting in 85-90% accuracy in endometriosis mapping, which requires a high level of expertise.
Also, Dr. Jay Mehta is a highly skilled & experienced fertility specialist & one of the best endometriosis specialist in Mumbai.
Treatment Options
Unfortunately, no medical management currently exists for Deep Endometriosis of the bladder. While some professionals may promise a cure, the reality is that due to the limited gland component, deep Endometriosis cannot be remedied with medication.
Temporary pain reduction can be achieved with Dinogest, Leupride, or GnRH Agonist Depot. However, individuals with Bladder Endometriosis should strongly consider a complete endometriosis excision by an expert.
Despite this, many young women in India are often misguided about treatment options and placed on medical management.
Surgery via 3D Laparoscopy is the only available treatment option for patients.
The challenge lies in reducing Adenomyosis and Endometriosis while keeping the tubes intact, requiring the expertise of surgeons.Â
If surgery is performed by an expert, it can enhance fertility, allowing patients with normal Fallopian tubes to conceive naturally.
Again, if performed by an expert, laparoscopic surgery significantly reduces the recurrence rate to less than 3%.
In our clinic, with the help of Dr. Jay Mehta & his team operates more than 500 cases of Deep Endometriosis across India annually.

Also, we observed some patients have this question in their mind, which surgery is better – Robotic Surgery OR Laparoscopic Surgery?
Currently, there is no proven benefit of performing Robotic Surgery over Laparoscopic Surgery for Endometriosis, even for deep lesions.
Despite this, 2-4% of patients opt for Robotic Surgery due to perceived safety. However, the literature demonstrates no additional benefits.
Post Surgery Recurrence
While Deep Endometriosis can be completely removed by the best surgical team, there is a chance of recurrence.
Post-surgery, most patients do not require any form of medical management if complete surgical resolution has been achieved.
At our clinic, Dr. Jay Mehta uses a specialized technique of doing this procedure with associated Radical Excision of the disease as well as complete clearance of the Bladder Endometriosis.
This technique of Dr Jay Mehta has been well documented as a Step by Step approach for treatment for Endometriosis as well as a Masterclass on Bladder Endometriosis. Dr. Jay has posted that video on YouTube.
In conclusion, Deep Endometriosis of the bladder is a complex condition that requires expert attention for diagnosis and treatment. Patients should be aware of their options and make informed decisions about their health care.

Focal Adenomyosis is most common in young women in their early reproductive years. The primary symptom is severe menstrual pain that progressively becomes resistant to medication.
While all medications prescribed for this condition, including painkillers, Dienogest, and Leupride injections, can provide temporary relief, they do not offer a permanent cure. These medicines work by stopping periods, thereby reducing the associated pain, but they don’t treat the underlying condition.
While it’s rare, Focal Adenomyosis can sometimes be painless. However, most patients experience significant discomfort, particularly during their menstrual cycle.
